New Patient Information
Parking:
Please park in the BLUE garage. Take the elevator to the hospital lobby. Then take the hospital elevators to the third floor. We are located in Suite 315 at the end of the hall.
We look forward to your upcoming appointment at the Center for Breast Health at Virginia Hospital Center. In order to make your appointment as productive as possible, please be sure to bring the following to your appointment:
1. Referral (if necessary)- if your plan requires a referral and you do not provide it on the day of service, you will be required to pay for your visit at the time of service.
2. Photo identification
3. Insurance information (primary and secondary)
4. Co-pay for a specialist
5. Completed new patient registration information
6. All mammogram, ultrasound, breast MRI reports and ACTUAL FILMS. If you do not bring your films, we may not be able to see you. If you have had you imaging done here at Virginia Hospital Center, you will not need to bring this since we are able to access the information on our computer system.
Please arrive 30 minutes prior to you appointment time so that we may transfer this information to your electronic medical record.
Follow-up visits:
Patients who are coming for a follow-up visit will be asked to inform us of any changes in medications or medical history. Please arrive 15 minutes prior to your appointment time so that we may update your electronic medical record.
Office Hours:
The office is open Monday through Thursday from 8:30-4:30PM, Friday 8:30-3:30. We do close the office from 12:00-1:00 for lunch.
If a physician is delayed due to an emergency, we make every effort to notify you prior to your appointment.
